Documents Required

  • Minor Road Traffic Accident (MRTA form) (for minor claims) or ROP report (in case of injury, fatality or third-party property damages)
  • Claim form (Own Damage & Third Party)
  • Copy of Driving License (Own Damage & Third Party drivers if any)
  • Copy of Mulkiya (Own Damage & Third Party vehicles if any)
  • Vehicle photos (Own Damage & Third Party vehicles if any)
  • Policy copy (Own Damage)

Payment of excess amount

AMI representative will contact you with the applicable excess amount within 1 working day.

Ways of Registering a Claim

Please upload the above listed documents in our online claims intimation portal or scan & send them to our email id motor.claims@almadinatakaful.com and our representative will be in touch with you within one working day.

Claim Structure

  • Claims will be settled within 14 working days from the date of the receipt of the complete documents
  • Direct settlement to member bank account

Life Claims 

  • Claims will be settled within 14 working days from the date of the receipt of the complete documents
  • Settlement via cheque to the policy holder.

    • Reimbursement form
    • Copy of medical report, lab investigation report
    • Prescription copy
    • Original payment receipt with itemized break down of availed services
  • Life
    • Death :  Claim Form, Valid ID copy, Salary certificate, Notification of death, Death Certificate, ROP report (accident), Medical reports (Sickness), Repatriation bills & Embalming certificate (Expatriates)
    • Disability :  Claim Form, Valid ID copy, Salary certificate, Compensation board certificate, ROP report (accident), Medical reports (Sickness)
    • Critical Illness :   Claim Form, Valid ID copy, Salary certificate, Medical reports

Documents Required

All the claims related to the above takaful products warrants following basic claim documentation for processing the claims which shall include but not limited to the following:

  • Claim intimation Note – by Verbal/Email/Fax
  • Duly Filled Claim form – Blank Form available on website/office
  • Evidence supporting Nature & Extent of loss/damage
  • Photographs depicting the loss/damage
  • Discharge voucher-cum-Letter of subrogation duly executed at the time of settlement
